Residential Program
Residents are adults with an IDD, who seek to build productive, meaningful and fulfilling lives in a unique residential setting.
APPLICATION PROCESS

The initial inquiry can be completed over the phone or via email with a Haven employee. This step in the process allows our team to assess whether an individual meets the minimum requirements for admission. If an individual does meet the minimum requirements, he or she will be given the opportunity to proceed to the next step of the process.

Following initial inquiries, the family, guardian and individual will re-visit the website to complete the application. The prospective resident’s application and supporting documents will be reviewed by members of The Haven team.

Upon vacancy, potential applicant will be contacted to re-determine level of interest and to discuss onboarding.

If the prospective resident appears to be a good fit, they will be invited to join the The Haven residents and team for dinner and outing. During the meal, team members will observe and notice peer to peer interactions as well as resident to team interactions.

Applicant will be offered placement in the residential program. Upon acceptance a leasing agreement form will be signed and first month’s payment will be due. The room move-in inspection will be completed during the meeting.

The prospective residents move into the The Haven for one to four weeks to experience the environment and participate in activities to ensure the resident is a good fit for the home. Upon successful first months move-in, residents are confirmed for long term care.

We accept private pay as well as med waiver residential rehabilitation funding.

The Haven is currently at full capacity. While housing for additional resident’s is not immediately available, it may be in the future. Inquire about joining The Haven waitlist by filling out the waitlist form. If there is availability, all applications will be reviewed, and a selection will be made based upon the best fit for the individual and our community.
Admissions Advisement
The Haven is not appropriate for individuals who have a psychiatric or serious emotional disturbance that impacts behavior, have serious medical conditions that require daily or weekly monitoring or intervention, and/or require 24-hour supervision.
